Women's Faster Biological Aging Tied to Higher Dementia Risk

A 26-year analysis of the Framingham Offspring Study has found that women whose bodies aged faster than their calendar age carried a doubled risk of dementia, a pattern not seen in men with the same accelerated aging profile. Researchers led by Pan, Beiser and Mitchell followed up to 3,046 participants, mean age 55 at baseline and 55% female, measuring a biological age estimate built from left ventricular mass, lung function, grip strength, gait speed, kidney function, cholesterol, glucose regulation and pulse wave velocity. Over a median follow-up of 23 to 26 years, women with accelerated biological aging had a hazard ratio of 1.8 for cardiovascular disease, 2.1 for dementia and 1.8 for mortality compared with those whose aging was slower than expected. Men with accelerated aging also faced higher cardiovascular and mortality risk but did not show the same additional dementia signal. The study was published in Scientific Reports on 12 August 2026.
Why the study cannot prove cause and effect
This is an observational cohort study, so it cannot establish that faster biological aging directly causes dementia; the authors state the design "precludes causal conclusions, and residual confounding cannot be ruled out." The Framingham Offspring Study population is predominantly white and US-based, which limits how far the pattern can be generalised to other populations, including the UK. The biological age measure combined multiple organ-system markers rather than a single blood test, and the paper calls for the sex-specific dementia finding to be validated prospectively in more diverse cohorts before it is treated as established. The article reviewed did not report a specific funding source for the analysis.
What this means for your routine
